At Madison Avenue Smiles, you have a variety of payment options. We accept:

Payment is requested after each office visit. While we do not accept direct assignment from insurance carriers, an experienced staff member will prepare insurance forms to be processed on your behalf, so that benefit payments can be refunded to you as quickly as possible.

You need only provide us with all pertinent information regarding your insurance and we can submit completed forms for you.

For extensive dental treatment, payment schedules are developed along with the treatment plans. Veronica, our Office Manager, can answer any questions regarding treatment time frames, payment schedules as well as other questions you may have.

We understand your time is valuable and we pride ourselves in keeping our schedule running on time. To make our scheduling system work, we need your support. Therefore, we request 24 hour advance notice if you need to cancel or reschedule an appointment. Multiple cancellations are subject to a cancellation fee.

Emergencies are always treated as a priority.

We appreciate your referrals; our appointment book is always open to any of your family members or friends.
